How Surrogate Compensation & Allowances Are Typically Paid

One of the most common questions Intended Parents and surrogates ask is: “How are fees and allowances actually paid during the journey?”

While every arrangement is customized and set out in the legal agreement, there are some typical patterns that help everyone plan and feel secure.

1. Base Compensation

The surrogate’s base compensation is usually divided into equal monthly installments during the pregnancy, often starting after a heartbeat is confirmed by ultrasound.

Typical timing examples (for illustration only):
• A small portion at legal clearance or embryo transfer
• The remaining amount split into monthly payments from heartbeat confirmation through delivery

The exact total and schedule will depend on the surrogate’s experience, state guidelines, clinic policies, and the Intended Parents’ budget—and will always be clearly written into the contract. Clear structure helps the surrogate know what to expect, and helps IPs plan their finances over the course of the journey.

2. Allowances and Reimbursements

In addition to base compensation, a surrogate typically receives:
• Monthly allowance for local travel, phone, parking, etc.
• Maternity clothing allowance at a certain week of pregnancy
• Reimbursement for pregnancy-related expenses (co-pays, mileage, childcare for appointments, etc.)
• Additional fees, if applicable, for procedures such as a C-section, invasive testing, lost wages, bed rest, or multiples.

Most programs use an escrow account so funds are deposited in advance by the Intended Parents and disbursed according to the agreed schedule. Escrow offers transparency: the surrogate can feel confident that her compensation is properly funded, and the IPs can see how and when payments are made.

3. Communication, Expectations & Legal Clarity

Compensation is not only a financial topic—it is also an emotional one. A thoughtful discussion about payments often opens the door to talking about expectations, work schedules, childcare, and how the surrogate’s daily life will be impacted.

Before any medical treatment begins, the surrogate, Intended Parents, and their separate attorneys review a detailed compensation schedule line by line. Questions like:
• When do payments start and end?
• Under what circumstances are extra fees triggered?
• How are missed appointments, bed rest, or travel handled?

are all addressed in writing. Only when everyone understands and agrees—amounts, timing, and conditions—is the contract signed and the journey moves forward.

4. One Key Principle: Mutually Agreed, Not One-Size-Fits-All

The most important point: There is no single “standard” plan that fits every family and every surrogate.

Each compensation and allowance structure should be openly discussed and mutually agreed between the IPs and the surrogate, with guidance from the agency and legal counsel. A clear, transparent plan honors the surrogate’s time and commitment and gives Intended Parents clarity and peace of mind—so both sides can focus on what matters most: a safe, healthy pregnancy and a much-wanted baby.
